PHC-4 is part of the Key Sources Strategy (KSS). PHC-4 stands for Prioritizing, Hubs, Collaboration, and 4 Priorities. The Prioritizing portion of PHC-4 involves proactive evaluation of the services and actions needed to address local and regional health care needs and put better systems in place to meet those needs. The Hubs portion of PHC-4 emphasizes the development of strategic partnerships to deliver health care services in the most cost-effective, efficient, and equitable manner possible. The Collaboration portion of PHC-4 is focused on the need for intersectoral collaboration to address health care needs. Finally, the 4 Priorities of PHC-4 are Patient Care, Community Engagement, Interprofessional Collaboration, and Health System Efficiency. PHC-4 sets out a comprehensive strategy to ensure that health care services are delivered in a comprehensive, equitable, and cost-effective manner to meet the needs of all Ontarians.
